Lot 3004667 was recalled due to reports of foreign matter contamination from multiple vaccination facilities. Lots 3004734 and 3004956, manufactured simultaneously using identical equipment but without contamination reports, were also voluntarily recalled. A new death case from Lot 3004734 was reported, bringing the total to three deaths. A 49-year-old male died the day after his second dose. The cause of death and causal relationship with vaccination remain under investigation.
