This page describes the application system for the Consumer Safety Investigation Commission. Consumers or any individual can apply to request accident investigation when they believe it is necessary to clarify the cause of consumer accidents involving injury or death. Applications are accepted in written form using the provided template and can be submitted by email or mail. Not all applications result in investigations; the Commission reviews each case and initiates investigation when deemed necessary. The Commission designates a contact officer for each case and provides information while considering victims' families' feelings. Inquiries about the application process should be directed to the Consumer Affairs Agency's Accident Investigation Division.
