自治体・医療機関等をつなぐ情報連携システム（Public Medical Hub：PMH）｜デジタル庁

This page describes the Public Medical Hub (PMH), an information exchange system developed by Japan's Digital Agency to connect municipalities, medical institutions, and pharmacies. PMH promotes digitalization in medical expense assistance, vaccinations, and maternal and child health using My Number Cards. Benefits include using the My Number Card as a health insurance certificate and medical assistance proof, and pre-filling questionnaires via smartphones. The system enables real-time access to vaccination records and health checkup results through the My Number Portal. Currently in pilot implementation with multiple municipalities, the system aims to expand coverage and integrate more services.
